We offer the following tests:

PCR – The most sensitive with results typically available within 24 hours of the lab receiving the sample.

Neutralizing Antibody – Offers certainty to your vaccine status. This test  measures the number of neutralizing antibodies in your system.

Rapid Antigen – Detects when individuals are infectious to others. Less sensitive than PCR but results available in 15-30 minutes from sample collection.

Rapid Antibody – Detects antibodies to SARS-CoV-2 virus from vaccination and previous or current infection. Results are available in 15-30 minutes from sample collection.

COVID-19 TESTING PATIENT CONSENT AND AGREEMENT

Rapid Biosense, LLC (referred to herein as “Lab,” “we,” “us,” and/or “our”) maintains the policies below with respect to COVID-19 testing (“COVID-19 Testing”). By signing this Patient Consent and Agreement, you agree to receive COVID-19 Testing from Lab and its employed, contracted, and affiliated healthcare providers.

  1. Informed Consent for COVID-19 Testing. 
    1. You voluntarily consent and authorize Lab to conduct collection, testing, and analysis for the purposes of a COVID-19 diagnostic laboratory test. 
    2. You acknowledge and understand that your COVID-19 diagnostic test will require the collection of an appropriate sample through a nasopharyngeal swab, oral swab, or other recommended collection procedures. 
    3. You understand and agree that there are risks and benefits associated with undergoing a diagnostic test for COVID-19 and, as with any medical test, there may be a potential for false positive or false negative test results. 
    4. You agree to assume complete and full responsibility to take appropriate action with regard to your test results. You acknowledge that a positive test result is an indication that you must self-isolate and/or wear a mask or face covering as directed in an effort to avoid infecting others. 
    5. You authorize your test results to be disclosed to the county, state, or to any other governmental entity as may be required by law. 
    6. You acknowledge and understand that the COVID-19 Testing equipment has not been approved by the U.S. Food and Drug Administration (“FDA”), but that it has been authorized by the FDA under an emergency use authorization for use by authorized  laboratories  and  patient  care settings, and  has been  authorized  only  for the  detection  of nucleic  acid from SARS-CoV-2, not for any other viruses or pathogens.
  1. Release and Indemnification. You acknowledge that the Lab is not performing a complete physical examination and is not liable for civil damages as a result of acts or omissions in performing COVID-19 Testing services. You hereby expressly, fully and forever agree to release, waive, indemnify, discharge, and hold harmless Lab, its owners, managers, investors, directors, officers, representatives, affiliates, employees, and contractors (collectively, the “Indemnified Parties”) from, and covenant not to sue any Indemnified Parties for, any and all liabilities, claims, losses, judgments, damages, costs, demands or causes of action, including attorney fees, that you may have at any time for any reason resulting from or related, indirectly or directly, in whole or in part, to services rendered by Lab or the disclosure of my COVID-19 test results regardless of any sole, concurrent or contributory negligence, acts, omissions or fault of Indemnified Parties. This Section shall survive termination or expiration of this Patient Consent and Agreement.

Your signature below evidences your voluntary agreement to undergo COVID-19 Testing services and that you are the patient or are authorized to act on behalf of the patient to sign this Patient Consent and Agreement. By signing below, you agree that you have read, understand, and agree to all of the statements contained in this Patient Consent and Agreement. Your signature below further evidences that you have been informed about the COVID-19 Testing services and the purpose, procedures, possible benefits and risks, and that you have been given an opportunity to ask questions before you sign and have been told that you can ask other questions at any time. You understand that your agreement is effective on the date signed below and that you may revoke your agreement in writing. Your revocation will not be effective for actions already taken by Lab or that are in progress and will only be prospectively effective. 

Patient HIPAA Authorization

AUTHORIZATION: I hereby authorize Rapid Biosense, LLC (the “Laboratory”) to disclose to [insert employer’s / school’s / athletic club’s name] and its employees and contractors (collectively, “Recipient”) information relating to my laboratory tests and results including, without limitation, my COVID-19 related testing and results, in any and all formats or portions now existing or hereafter concerned (examples of formats are paper, digital, internet). 

This authorization will remain in effect for two (2) years after the date I discontinue receiving services from the Laboratory.  I understand that I have the right to revoke this authorization, in writing, at any time, except where uses and disclosures have already been made based upon my original consent.  Revocation must be communicated in writing to the Laboratory.

I understand that authorizing the disclosure of this information is voluntary.  I also understand that I may refuse to sign this authorization and that my refusal to sign will not affect my ability to obtain treatment, payment for services, or eligibility for benefits unless the information is necessary to demonstrate that I meet eligibility or enrollment criteria.  I understand I am entitled to a copy of this document in its complete form.  I understand that it is possible that information used or disclosed pursuant to this consent may be subject to re-disclosure by the recipient and may no longer be protected by federal or state law.

FAQ

What are the different test types?

We offer COVID-19 testing via rtPCR, neutralizing antibody, rapid antigen, and rapid antibody. All four test types are offered at the USA Hockey Arena location Plymouth, MI, or through our On-Site Collection services program here. Currently, only PCR-based testing is available at our pharmacy collection sites with results available typically in 12-24 hours from receiving the samples at our lab.

What is a PCR-based test?

PCR, also known as rtPCR, is the most sensitive and accurate test and involves polymerase chain reaction to detect nucleic acids of the virus which will determine whether a patient is currently infected or in the early or late stages of infection. Results are typically available in 12-24 hours from receipt of sample at the lab.

What is a neutralizing antibody test?

Neutralizing antibody test measures the amount of antibodies you have in your system. This test offers certainty  certainty to your vaccine status. Turn around time is 12-24 hours.

What is a Rapid Antigen test?

Rapid antigen detects a protein on the virus and determines if a patient is currently contagious and actively shedding the virus. The test is less sensitive than PCR and not as accurate. Results are typically available in 15-30 minutes from providing a sample.

What is a Rapid Antibody test?

Rapid antibody detects for two different COVID-related antibodies in the blood and determines whether you have antibodies against the SARS-CoV-2 virus, either from vaccination or prior/current infection. Results are typically available in 15-30 minutes from providing a sample.

What is the turnaround time?

  • The typical turnaround time for our PCR test is within 24 hours of your sample arriving at our facility.
  • Rapid Antigen and Rapid Antibody results typically take approximately 15-30 minutes from sample collection on-site.

We also offer priority processing of samples for a fee with a guaranteed turnaround time listed on the website. Priority is typically only available at our drive-through site at the USA Hockey Arena in Plymouth, MI but may be coordinated at times by contacting customer service.

How much does testing cost?

For American citizens with health insurance, the rtPCR and rapid antigen tests are covered in full. Even without insurance, or if your insurance provider denies coverage, you will never be billed for your test, according to the CARES Act through the United States federal government.

For travel-related testing, you should confirm with your insurance company if this is covered. If not, you can select our credit card payment option for testing.

When are you open?

Hours vary by sample collection location and can be found at our website here.

Our main testing site at the USA Hockey Arena is open for testing Monday through Friday 10:00am to 8:00pm, and Saturday through Sunday 9:00am to 1:00pm.

Do I need an appointment?

We highly recommend registering for an appointment ahead of time at our website here.

If you cannot make an appointment in advance, you will need to register on your mobile device on-site prior to your sample collection for the fastest service. If you are unable to pre-register or have issues with the process, please contact customer service prior to arriving for testing.

Do your tests and result reports fulfill all of the requirements for international travel? What about for travel to Hawaii?

Yes, the information included in your report (your test type, dates of collection and reporting, CLIA ID, passport number if applicable, etc) fulfills all requirements for international travel.

Please note, certain destinations require a specific type of sample collection, ie a nasopharyngeal swab (NPS), and is the responsibility of the client to know what is required. Our typical method is an anterior nasal swab (ANS), which is widely accepted, but can provide NPS when required.

If you are booking a test for travel to Hawaii, you will need to book your appointment through AZOVA and select one of our locations on their site.

Where are you located?

Our main testing site is located in the parking lot of the USA Hockey Arena on Beck Rd in Plymouth, MI. This site offers PCR, Rapid Antigen and Rapid Antibody tests for COVID-19.
